Health Insurance Plans

PPO Plans
A PPO plan (preferred provider organization) is a managed care system under which a network of doctors and hospitals provide services to the plan participants at a discounted rate. Participants are permitted to see a physician who is outside of the PPO network, but they will be responsible for a larger amount of the cost for the services rendered.
HSA Plans
An HSA plan (health savings account) works like an IRA, except that the money is used to pay for health care costs. Plan participants must enroll in a High Deductible Health Plan (HDHP) in order to be eligible to fund an HSA to cover current and future medical expenses. All money is tax deductible when deposited, and earnings are tax deferred. The money can be withdrawn to cover any qualified medical expense without being taxed. The unused balance can be rolled over from year to year to be used for future healthcare or long-term care costs.
HRA Plans
HRA plans (Health Reimbursement Arrangement) are IRS regulated, employer funded medical expense reimbursement plan from Section 105 of the IRS Code. An HRA plan can be combined with a HDHP (high deductible health plan) or can be purchased as a stand alone plan. Under this type of plan, the employer funds an account for each employee that can be carried over at the end of each plan year for use in future years, or can be returned to the employer, based on the plan design.
Funding Choices
There may be several choices available for funding the cost of health insurance, depending on the size of your group. These range from fully insured plans with set rates, to insured plans with a low to moderate level of risk sharing on the part of the employer, to Administrative Services Only (ASO) plans under which claims are self-funded by the employer with the exception of high claims that are covered under a stop-loss insurance policy.

Section 125 Plans
A section 125 plan allows employers to offer their employees a variety of benefits on a pre-tax basis. There are four main parts to a Section 125 Plan.
Premium Only Plans – A premium only plan permits an employee to have qualified insurance premiums deducted from his/her paycheck without federal, state, local, Medicare or Social Security taxes.
Flexible Spending Accounts – A flexible spending account allows an employee to have money deducted from his or her paycheck on a pre-tax basis to help cover out of pocket medical, dental, vision and prescription (some over the counter) expenses for themselves and their family members throughout that calendar year.
Funding Dependent Care Spending Accounts – The dependent care spending account portion of the Section 125 Plan allows an employee to deduct child care expenses from his/her paychecks on a pre-tax basis.
Transportation Benefits – A transportation benefit program is a part of the Section 125 Plan that permits an employee to deduct parking fees, transit passes and certain other necessary transportation expenses from their paycheck on a pre-tax basis.
